To become an active volunteer you need to submit the following documentation: copy of car insurance policy card, and signed waiver. (see back of form for waiver.)
The Kenwood Wildlife Center would like to welcome its volunteers. What you are doing is of great importance to all forms of life as well as to the health of the earth itself. It is important to let you know that there are some risks involved in our work. These risks include, but are not limited to, the spread of parasites of diseases, physical injury or damage to property that occurs during the transportation of sick or injured animals to and from the KWLC, or damage from toxins.
Although the probability of any of the above happening to you is remote, we want you to feel free to ask any questions that you may have about our work or the possible risks involved. We encourage you to decline any assignment which does not feel safe to you.
Because the Kenwood Wildlife Center cannot assume the liability for these risks, we ask that you complete and hand in the following release:
ASSUMPTION OF RISKS AND RELEASE OF LIABILITY
I UNDERSTAND THAT THERE ARE SOME RISKS CONNECTED WITH MY WORK AS A VOLUNTEER FOR THE KWLC, AND THESE RISKS HAVE BEEN OUTLINED TO ME. I HEREBY ASSUME THE RISKS AND WAIVE, RELEASE AND DISCHARGE ALL CLAIMS AND CAUSES OF ACTION FOR LIABILITY AGAINST THE KKENWOOD WILDLIFE CENTER, ITS BOARD MEMBERS, OFFICERS AND VOLUNTEERS, FOR ANY INJURY, ILLNESS, DEATH OR LOSS I HAVE OR MAY HAVE IN THE FUTURE, FROM ANY CAUSE, AS A RESULT OF MY PARTICIPATION AS A VOLUNTEER FOR THE KENWOOD WILDLIFE CENTER.
I AM AT LEAST EIGHTEEN YEARS OF AGE. I HAVE READ THIS RELEASE CAREFULLY AND FULLY UNDERSTAND ITS CONTENTS. I REALIZE THAT I AM RELEASING CERTAIN LEGAL RIGHTS THAT I MAY OTHERWISE HAVE, AND I SIGN THIS DOCUMENT FREELY AND VOLUNTARILY.
